Medical Examination Report Forms

There are two distinct Request for Medical Examination Report Forms:

Medical assessment NT12A (Word)

This form is to assess if an employee is unable to perform their duties at the requested level due to a medical condition.

Medical incapacity NT12B (Word)

This form is for the medical assessment of an employee who may be totally and permanently incapacitated.

Medical Examination Purpose

For what purpose is this medical examination being organised?

  • invalidity retirement
  • fitness to carry out current duties or other duties
  • fitness to continue employment
  • fitness to return to duty
  • identify medical limitations
  • inappropriate or long term use of sick leave
