Medical Examination Report Forms
There are two distinct Request for Medical Examination Report Forms:
Medical assessment NT12A (Word)
This form is to assess if an employee is unable to perform their duties at the requested level due to a medical condition.
Medical incapacity NT12B (Word)
This form is for the medical assessment of an employee who may be totally and permanently incapacitated.
Medical Examination Purpose

- invalidity retirement
- fitness to carry out current duties or other duties
- fitness to continue employment
- fitness to return to duty
- identify medical limitations
- inappropriate or long term use of sick leave
